(deze ontvangt u per e-mail).<\/li>\n<li>Identiteitsbewijs van <strong>uw kind.<\/strong><\/li>\n<li>Ontwikkelingsperspectiefplan (indien van toepassing).<\/li>\n<li>Relevante rapportage(s) van onderzoek(en) en\/of een diagnose op medisch, psychiatrisch, cognitief, sociaal, emotioneel en\/of didactisch gebied (indien van toepassing).<\/li>\n<\/ul>\n<p>De gesprekken zijn op: donderdag 7 mei,
